Select one of the chronic diseases listed below. In each of the assignments in this course, you will address the chosen chronic disease as the topic.

You must cite at least two scholarly sources. This paper should be about 4-5 pages long.

You will do the following in Assignment One:

Draw a population diagram. Who are the people who make up this group? Give specifics, such as the disease’s contributing causes and/or past medical history, as well as any familial or genetic components.
Evaluate the population, including current numbers, disease severity, and special needs, in order to optimize health and independence.
Determine whether formal case management is required. What are the current trends in case management that support a case management model (as discussed in your text)?
Argue the potential benefits of implementing a case management model, addressing economics, patient quality of life/care, and impact on the patient’s and family’s social lives.
Examine why nurses should collaborate on the case management plan of care. What specialized training and experiences would a nurse bring to this role, based on best practices?
Identify the other members who should be on a multidisciplinary case management team and discuss their roles and why they are important members.
