Writing about one’s personal experiences promotes self-reflection and self-awareness. In this case,

In this activity, students are asked to write about their experiences managing clinical risks for patients. Students are asked to write about an event that they witnessed or were a part of during their nursing career that they believe put a patient at risk. Students may also choose to write about an event that occurred to them as recipients of

healthcare, or as a bystander to the experiences of their friends or family members. The event could be related to any of the following: anticipating, recognizing, or managing a patient safety risk. Nurse practitioners must be aware of potential risks and how to avoid them in order to provide safe patient care and reduce medical-legal risk.

The following information should be included in the narrative:

a description of the event an explanation of how this event put a patient at risk the student’s reflections on the event including: if and how the event was recognized why and how the event occurred, for example, was the event caused by an individual healthcare provider’s action or inaction system failure(s) a combination of system failure(s) and individual provider performance issues
what changes or enhancements they would make

make, if any, a statement about how they felt at the time and how they currently feel about the event
Were you a participant in the event? Discuss.
What, if any, workplace factors played a role in the occurrence? (For example, was it during a handover? Was there medication involved?)
How well did the healthcare provider(s) understand what was going on around them? (awareness of the situation)
How did the health care provider(s) recognize and respond to the safety issue?
What impact did this event have on you?
What did you learn that you will put to use in the future?
