Consider the following:

Describe the communicable disease (causes, symptoms, mode of transmission, complications, and treatment) as well as the target demographic (mortality, morbidity, incidence, and prevalence). Is this a disease that needs to be reported? If so, include information such as reporting time, who to report to, and so on.
Describe the health determinants and how they contribute to the development of this disease.
Examine the epidemiologic triangle in relation to the communicable disease you’ve chosen. Include host and agent factors (presence or absence), as well as environmental factors. (Each element of the epidemiologic triangle is described in the textbook.) Is there anything special that the community, schools, or general population should be aware of?
Describe the role of a community health nurse (case finding, reporting, data collecting, data analysis, and follow-up).
Identify at least one national agency or organization that addresses the communicable disease of choice, and explain how the organization(s) contributes to disease resolution or reduction.
Discuss the disease’s global implications. What is the response in other countries or cultures? Is this disease endemic to a specific region? Give an example.
