Nursing problems are frequently derived from a source of concern or a gap in clinical practice or theory. The student should address a problem or issue related to supporting health, providing care, or interacting with a variety of providers and/or patients in order to complete the clinical practicum experience. Observing nursing care in a clinical setting and researching current issues related to nursing and the health care profession in a peer-reviewed journal are examples of problem-solving methods.

Create a two- to three-page paper describing the following:

Determine a problem that is relevant to your clinical setting. (There are frequent falls. The eleventh of October is a leap year.
Determine the PICOT question and the null hypothesis. My PICOT question is: In a high-risk fall patient on a Medical-Surgical unit, how does assigning staff to sit with patients around the clock prevent falls during their hospitalization versus hourly rounding?
Describe the issue’s history and context.
Describe current evidence-based practice research on the subject.
Determine your stakeholders.
Summarize the inherent costs of this issue to patients, healthcare organizations, healthcare providers, and so on.
