Please include citations and references in APA format to back up your thoughts and opinions. Substantive means that you contribute something new to the discussion rather than simply agreeing. This is also an opportunity to ask questions or provide information about the topic covered by your peers. Personal experience is appropriate for a substantive discussion and should be supported by research. Review your APA errors in your reference list, especially the capitalization errors in some of the titles. Include the DOI as well. Also, make certain that the titles of online sources are italicized. There should be no more than 200 words.

Transitional care is defined by the American Geriatrics Society (2003) as “a set of actions designed to ensure the coordination and continuity of health care as patients transfer between different locations or different levels of care within the same location” (Cited in Boltz, et. al., 2012). It also includes the sending and receiving aspects of the transfer and is based on a comprehensive plan of care that includes logistical arrangements, patient and family education, and coordination among the health professionals involved in the transition†(Coleman & Boult, 2003 cited in Boltz, et. al., 2012). Finally, it entails a wide range of services and environments designed to promote the safe and timely transition of patients between levels of health care and across care settings†(Naylor & Keating, 2008 cited in Boltz, et. al., 2012). As a result, transitional care is an essential component of managing critically ill geriatric patients. This care will be expected of you.

Choose five research articles that address Transitional Care of the Geriatric Client and briefly describe at least three strategies you will use in your practice to reduce the negative effects of transitional care.
