Explain the theorist (short biography and major influences on the theorist).
Indicate the theoretical category in which the theorist falls (can use McEwen & Wills classification or other source).
Incorporate the theory’s underlying assumptions (including how metaparadigm concepts are defined).
Include the theory’s main concepts.
Include the theory’s major propositions.
Give examples of how the theory has been applied in the literature.
Explain how the theory will be put into practice at your institution (or how it will be used as a foundation, framework, etc.). This is a plan of action.
Give concrete examples of how theory would be integrated (for example, an assessment guide) and what patient outcomes could be measured as a result of implementing the theory.
Conclusion: Contrast current practice with what would happen if theory guided practice.
